At the start of the video game, the protagonist regains consciousness and engages into a challenge of freeing himself from the imprisonment in an asylum set up by Jigsaw Killer. The protagonist will soon find himself subjected to the gruesome treatment that has characterized Jigsaw Killer. Self-mutilation, suicide and murder are the three types of treatment in Jigsaw Killer's asylum.
If the player fails to press quickly the correct buttons, a metallic helmet might bisect the cranium of Detective Tapp. Other bottleneck challenges and deadly confinement areas also make the game more exciting. Each wrong step can also lead to fatal consequences.
